The Postural Care Pathway (PCP) is a service provided 3C that ensures all seating medical devices continue to meet the clinical needs of the client.
The PCP is based on an equipment servicing framework that aims to ensure reliable performance of our seating systems for the client. The servicing intervals provide preventative maintenance measures that reduce the risk of breakdowns and urgent repairs.
The PCP develops detailed insights to client developmental needs. These are evaluated with the relevant clinicians and therapists to promptly address any identified postural seating need.
PCP aims to provide a continuous review of our postural equipment and clinical support to enable our clients to be healthy, comfortable and maximise their functional potential.
PCP ensures the Case Manager, everyone in the MDT, can directly report wheelchair and seating concerns simply and easily.
PCP has a planned maintenance programme for all 3C issued wheelchairs.
This provides a simple, one-point of contact for any aspect or part of the seating needs.
PCP is an annual program that allows all stakeholders to analyse past performance and future needs.
Effective analysis can help improve the expectations on client outcomes through well-defined identified data driven seating needs.
