The Postural Care Pathway (PCP) is a service provided 3C to ensure that all seating medical devices continue to meet the clinical needs of the client.
The PCP is managed by a qualified clinician that will be in regular contact to monitor the postural seating performance and any clinical developments that arise over the year. PCP includes regular servicing of your 3C seating products and allows our qualified engineers to detail any wear or damage that may impact the overall performance of the seating.
PCP aims to provide a continuous review of our postural equipment and clinical support to enable our clients to be healthy, comfortable and maximise their functional potential.
PCP ensures the Case Manager, everyone in the MDT, can directly report wheelchair and seating concerns simply and easily. These are documented using a validated SIT tool and shared via an encrypted data portal – Doclend.
PCP has a planned maintenance programme for all 3C issued wheelchairs. This provides a service frequency that maintains and repairs wheelchairs and seating equipment in a responsive, timely and effective manner.
PCP includes an annual report that provides a summary of the current postural seating system performance and how this may be adapted in future, allowing the Case Manager to update any postural management strategies.
